The Direct Support Professional Relief/ Floater position, also referred to as DSW, requires mental maturity. The position requires individual to have the ability to teach. The heart to nurture and provide care for the individual you are blessed to serve. The position requires role modeling. The DSW must exhibit the ability to listen and follow directions for care of others. It requires knowledge of the general care and training of physically and mentally disabled people as well as elderly individuals with health issues. Health care experience is preferred, but not required. You must demonstrate competency or have verifiable work experience in providing support to individuals with disabilities. Training in human services includes but is not limited to Home Health Aide or Certified Nursing Assistant or Verifiable and documented work experience providing direct care services to the targeted population.

What is a Relief/ Floater
You are being hired as a relief/ floater DSW, meaning you will be called upon to relieve primary workers in the event of an emergency or to fill gaps when primary workers request off. As a relief/ floater you are required to call the office at 681-9988 daily to check in with administration. At times administration will attempt to contact you for work. After three failed attempts we will deem that you are not interested in being employed with People First Outreach and you will be terminated from the position. As a relief/ floater you are expected to be available often. If you have other employment please notify the office ahead of time with your schedule. You will often be called upon to work with short or no notice. There are no guaranteed hours as a relief/ floater. You ultimately determine how much you work. Your determination and hard work is easily identifiable and will be paid off with steady work.
Requirements of a DSW
- Must be able to work flexible shifts on weekends.
- Must be able to work without direct supervision and demonstrate initiative, empathy, tolerance, consistency and motivation when interacting with clients.
- Must demonstrate appropriate dress and behavior (scrub set- top and bottom- with closed toe rubber sole shoes)
- Follow the policy of People First Outreach regarding the documentation of time sheets, service logs, incident reports, seizure forms, critical incidents and physician referral forms.
- Submit accurate and complete documentation each week by midnight Saturday.

- No more than two (2) moving violations within the past three (3) years, no at-fault accidents within the past three (3) years, and no convictions of Driving Under the Influence (DUI), Reckless Driving, Driving While Intoxicated (DWI), Vehicular Manslaughter, Driving Dangerously, or other similar type of offense.
Job Description
- Read and understand the Comprehensive Plan of Care provided by the State of Louisiana or the consumer’s Support Coordinator Agency along with People First Outreach’s Provider Plan of Care.
- Maintain weekly training with the consumer on the execution of the Emergency Plan and Evacuation Procedures. All training must be properly documented.
- The job includes but is not limited to assisting individuals in the following areas and following the procedures of People First Outreach:
- Universal Precautions; Hand washing, wearing gloves.
- Use LASRS system to clock in and out of the client’s home.
- Assist client with all areas of daily living; included but not limited to bathing, grooming, dressing, ambulation, eating, transferring, and toileting.
- Activities of Daily Living such as light housekeeping, emptying trash can, food preparation and storage, grocery shopping, laundry, medication reminders, accompanying to medical appointments, assistance with arranging medical transportation and assistance with scheduling medical appointments.
- Demonstrate empathy toward the elderly and persons with disabilities,
- Have an ability to provide care to the recipient, and the maturity and ability to deal effectively with the demands of the job.
- Provide services outlined in CPOC Comprehensive Plan of Care and the PFO Provider Plan of Care to prevent out of home placement
- Document services provided on appropriate service log, and time sheet. Submit documentation weekly by Saturday midnight.
- Notify People First Outreach (available 24 hours a day) when you are going to be late or unable to work scheduled work shift.
- Advise People First Outreach Office of problems, concerns, and triumphs.
- When applicable (with doctor’s authorization), administer medication and document.
- Encourage and assist with recreational activities where applicable.
- Prepare and serve food.
- Report violations and abuse allegations, to parent and the office of People First Outreach.
- Report Critical Incidents at the time of discovery to People First Outreach available 24 hours a day.
- Transport client when needed; People First Outreach office should always be contacted first before your car moves we are not required to transport all clients.
- To maintain client confidentiality and the confidentiality of the client‘s family.
- To comply with rules/regulations according to agency guidelines, state and federal licensing and certification requirements.
- To practice preventive safety and health techniques.
- Carry out trash.
- Be respectful to client and client’s family
- Staying off the client’s phone.
- Not using your cell phone while at work; never use your cell phone while driving a client.
- Weekly fire, emergency and evacuation procedures and documentation of such.
- And any other duties as deemed necessary by the People First Outreach and the family.
